as many of our patients have some form of dental insurance, we will be glad to complete the treatment portion of the dental insurance form. if your insurance company allows you to assign the benefit to our office and you have all your information, as a courtesy to you, we will accept it with certain conditions, which our business assistants will gladly go over with you prior to your appointment.
